Health questionnaires which are emailed to Boxmoor Yoga are printed and emails deleted. Printed questionnaires are stored securely.

 

Boxmoor Yoga does not share any of your information with any other organisations or process your data in any way.

 

Details on your health conditions, contact details and emergency contact details are stored in printed form and locked away. This information is collected so that I can make necessary adjustments for you in class, contact you about the classes you attend, and contact someone in the rare event of a medical emergency.

 

Health questionnaires for individuals who have not attended classes for three years or longer will be securely destroyed on an annual basis. You can request to have your details destroyed at any time.
